Fredonia Place (Community Healthcore)

Fredonia Place is an 8 bed co-ed facility and 2 unit apartment scattered site transitional housing program for individuals who are homeless and diagnosed with severe and persistent mental illness.  Its function is to provide homeless individuals disabled by mental illness a temporary place to reside while they engage in strategically linked treatment/support services.  Fredonia Place has 3 primary Program Goals in helping residents: 1) Obtain and remain in permanent housing; 2) Achieve greater self determination; and 3) Increase skills and/or income.
